Status Glossary:

  • Inactive & Non-Renewed – Licensee is not allowed to practice.
  • Probation – Licensee is monitored while complying with Board requirements.
  • Relinquished, Revoked, Surrendered, Suspended – Licensee is not allowed to practice.
  • Relinquished is non-disciplinary. Probation, Revocation, Surrender, Suspension are disciplinary.
  • If no date appears next to Expiration Date, it is not applicable.
