Request for License Verification

Description

A Licensee can request the Board provide an official signed and sealed document verifying the status of her/his license. This document will be sent to the entity (another licensing board or other relevant organization) listed on the License Verification Request Form . The fee for this service is $50.

To initiate the process, the form can be submitted to the Board digitally using PDF annotating software (e.g., Adobe Acrobat) or downloaded and completed manually.   If completed digitally, the application can be submitted by: (1) uploading it to this portal, or (2) emailing it to mdh.acupuncture@maryland.gov.  If completed manually, the application can be submitted by: (1) scanning to your device and emailing it to mdh.acupuncture@maryland.gov; or (2) faxing the document to (410) 431-3072.

Payment is accepted in this portal only via Master or Visa credit/debit cards. 

NOTE: If payment is made prior to the form being submitted, the License Verification will not be processed until the required form is submitted.

If documents are listed above, they are generally required for this service. Additional documentation may be requested by the Board when necessary.
