Servicemember Portability License Application

 Description

This application is for chiropractors who are Servicemembers or spouses of active Servicemembers and are seeking temporary recognition to practice in Maryland under the federal Veterans Auto and Education Improvement Act of 2022 (PL 117-333).

Applicants must already hold a chiropractic license in good standing in another state and have actively practiced within the past two years. Recognition is limited to the duration of the military assignment and Maryland residency. This form is not for a permanent license.

Estimated Time to Complete: 15–20 minutes, assuming all supporting documentation is available.

 Required Documentation*

Documentation to be sent by the applicant or license/registration holder:
  • Completed .pdf Application (including all signed statements in .pdf)
  • One (1) Passport-Size Photo (2" x 2")
  • CPR Card (Unexpired & Healthcare Provider Level)
  • Military ID of spouse and Copy of Marriage Certificate (If Applicable)
Documentation to be sent from a third party:‑
  • Two (2) Moral Character References
  • Verification of Good Standing from out of state Board(s) (If Applicable)

‑Third-party documents must be sent directly to the Board by someone else (such as a school, reference, or another licensing agency). You are responsible for requesting them and confirming they are submitted. They may be sent using the instructions in the confirmation email or emailed to: mdh.chiropractic@maryland.gov.
