Complaint Form

 Description

This form is used to file a formal complaint against a licensed chiropractor or registered chiropractic assistant (CA) in Maryland. The Board reviews all complaints to determine whether there has been a violation of the Maryland Chiropractic Act.

Complainants should provide detailed information about the individual involved, the nature of the complaint, relevant dates, and any witnesses. Supporting documents may be attached. The Board cannot investigate billing disputes or issues outside its authority.

Estimated Time to Complete: 15–20 minutes, depending on the complexity of the complaint and supporting information.
